TERMS & CONDITIONS

1. Bank account direct debits incur a $0.88 admin fee. Credit card direct debits incur a $1.11 admin fee.
2. Failed direct debit payments will incur a fee of $15 per failed debit.
3. 12-month upfront memberships have a 6week free suspension period.
4. *Unused suspension is not carried over to new memberships.
5. 6-month direct debit contract memberships have 4 week free suspension within the first 6 months of contract.
6. 6-month direct debit contract memberships will continue to debit once first 6 months have completed unless notified.
7. 4weeks notice must be provided when cancelling direct debit memberships.
8. 12-month upfront memberships are non-refundable.
9. It is on the onus of the member to inform 5D Health and Fitness Pty Ltd or 5D Health and Fitness Mitcham Pty Ltd of any new health conditions that may inhibit the health and safety of themselves or others in the facility.
10. 5D Health and Fitness Pty Ltd and 5D Health and Fitness Mitcham Pty Ltd has the right to terminate any membership at its’ discretion.
11. 'Stealing' and 'Damage' of equipment will be referred directly to Police with appropriate action taken.
12. 5D Health and Fitness Pty Ltd and 5D Health and Fitness Mitcham Pty Ltd accepts no liability for any injury sustained howsoever whether caused by negligence of the centre, it’s staff and patrons or otherwise.
13. As a member I agree to release indemnity and keep harmless from the business from all claims for loss of property from lockers or otherwise and for personal injury together with any associated costs or expenses.
